SAN FRANCISCO AHEC

The  San Francisco Community Clinic Consortium (SFCCC) is the San Francisco AHEC. SFCCC is a partnership of 12 community health centers that together form the largest provider of primary care safety net services in San Francisco.

SFCCC implements the AHEC Scholars in Community Health Program, which includes providing supplemental health education to health professions students to encourage them to explore careers in community care. SF AHEC Scholars are challenged to “challenge” their perceptions of community health by exploring the activities that create the fabric of the social and health services safety net in San Francisco.

SFCCC hosts the National Health Corps Program, which advances community health by providing critical education, care coordination, and essential services, while transforming systems through policy, procedure, and practice change.  National Health Corps members are committed to a year-long service-learning program, working as outreach, health educators, and capacity builders in member clinics; shadowing health professionals in clinics, community-based organizations, and more. A majority of the members go on to be trained as health professionals and are interested in working with clinics and health center-served populations.

SFCCC also arranges rotations for primary care residents and nursing students in community health settings and provides continuing education programs for practicing health professionals. SFCCC sponsors a Health Care for the Homeless program, Street Outreach Services, that provides outreach, health education and psycho-social support to the city’s unhoused community by partnering with volunteer health professionals, medical residents, nursing students, and others. To date, the program has trained hundreds of UCSF primary care medical residents, UCSF PRIME medical students, and numerous others looking to expand their clinical experience. After SOS rotations, residents and medical students recognize and overcome barriers to healthcare services for the unhoused population; recognize and treat medical and psycho-social issues of the unhoused population; and successfully work with a multi-disciplinary team, including outreach workers and case managers. After completing rotations, all participants have stated that the experience reinforced their interest in primary care and in working with medically disadvantaged urban populations.

While not all programs offered at SFCCC are supported by AHEC, SFCCC offers a variety of programs targeting different areas of healthcare to create a stronger support system for all.
